vRestricted work or job transfer
vMedical treatment beyond first aid
vLoss of consciousness
vA significant injury or illness
diagnosed by a physician or other licensed health care professional

vOSHA 300 form is the log of work
related injuries and illnesses

vOSHA 300 –A is the summary of work
related injuries and illnesses

vOSHA FORM 301 or equipment is the
worker’s and employer’s report of occupational injury or disease

vYou must involve your employees and
their representatives in the record keeping system
